Order can be placed by one of the following options
PHONE:
You can place an order over the phone by using a credit card, COD
($10.00 delivery charge) or you may apply for an open account so we
may send you an invoice. We will arrange to ship you the appropriate
kits and paperwork for your sample collection.
ON-LINE:
Complete the Company Profile Form, specify the tests you would like to
order in the comments box at the end of the form and click submit. We
will call you to process your order by obtaining a credit card number.
(Live ordering on line is not currently
available for businesses.)
